An initial task force inquiry into the fatal Teluk Intan road accident, which claimed the lives of nine Federal Reserve Unit (FRU) personnel last month, found the tipper lorry was severely overloaded.

It specified that the lorry was transporting 40,960 kg of gravel during the incident, exceeding the 24,000 kg authorized limit by 70.67%.

According to the report published on the Ministry Of Transport’s (MOT) website yesterday, the truck, operated by Tashveen Trading failed the JPJ’s safety audit and inspection miserably, scoring a mere 15% out of a possible 70% and falling short on all seven essential criteria.

“Tashveen Trading was also operating without a required safety supervisor, a clear breach of Department of Occupational Safety and Health regulations,” the report read.

Regarding the FRU truck, the report highlighted critical safety concerns with the vehicle, specifically noting the lack of seat belts for both front and rear passengers, as well as a similar absence of headrests in the rear seating area.

“The absence of these safety features significantly increases the risk of injury and death,” it says, adding that the lorry was also found transporting both goods and passengers in its rear, which heightened the risk of passenger injury during a collision.

Further investigations are ongoing to identify additional factors that may have contributed, including those related to human error, organizational structure, vehicle condition, and other significant aspects.

Just last month, APAD revoked the operating license of Tashveen Trading. This action was taken after the Agency found numerous safety breaches committed by the company, notably transporting goods not authorized under their vehicle permit.
