APAD Demands Answers: Prasarana Ordered To Submit Urgent Report Following Scary LRT Derailment In KL

Hearing the word "derailment" on your morning commute is genuinely every transit user's absolute worst nightmare.
Yesterday morning, an Ampang/Sri Petaling LRT line train slipped off its tracks near the Chan Sow Lin station due to a faulty track switch. While the incident caused immediate service disruptions and left peak-hour commuters scrambling, there is a massive collective sigh of relief: all 25 passengers on board were evacuated safely and completely unharmed.
However, the authorities are not letting this slide as just another minor technical glitch.
The Land Public Transport Agency (APAD) has officially stepped in, directing operator Prasarana Malaysia Bhd to submit a comprehensive report immediately so they can find out exactly what went wrong behind the scenes.
Independent Probe Opened: Negligence Is On The Table

Acting on a direct order from Transport Minister Anthony Loke, APAD has wasted no time and has immediately opened an official investigation paper to launch a thorough, top-to-bottom probe.
This isn't just a routine paperwork check. APAD has explicitly stated that the independent investigation will seek to establish whether there were elements of:
- Non-compliance with standard operating procedures (SOPs)
- Negligence by any individuals performing duties
- Breaches of official operating licence conditions
- Failures in asset maintenance and train safety systems
To show how seriously the government is taking this, back-to-back emergency site inspections were carried out on the very same day. The first rush to the scene was led by APAD Director-General Ahmad Radhi Maarof, followed by a second 3:00 PM inspection joined by representatives from the Ministry of Transport (MOT) to review initial operational safety.
"No Compromise" — Prasarana Could Face Serious Legal Action
If you think this will just result in a slap on the wrist, think again. APAD has made its stance crystal clear: passenger safety, the reliability of the rail system, and the integrity of KL's public transport network cannot be compromised under any circumstances.
If the investigation uncovers any corporate negligence or failure in personal duties, APAD is fully prepared to take strict legal action under the Land Public Transport Act 2010 [Act 715]. The authorities are zooming in on Section 101 to hit the operator with serious corporate penalties for breaching licence conditions, alongside Sections 128 and 202 to enforce direct legal action against any specific individuals whose negligence compromised rail safety.
While track switch malfunctions can happen, the public expects a flawless maintenance culture when millions rely on these trains daily. APAD has promised that this investigation will be carried out completely independently, transparently, and professionally to restore public confidence.
